Tandem Care is hiring a Care Coordinator who thrives in fast-paced, high-volume home care environments and has real experience working directly with caregivers and clients.
This role sits at the intersection of client experience, caregiver support, and care planning. You’ll be on the phone, in systems, and communicating constantly — reviewing care needs, coaching caregivers, addressing issues as they come up, and making sure care plans actually work in the real world.
This is not a passive, intake-only, or administrative role. If you’re energized by problem-solving, juggling multiple clients at once, and making judgment calls throughout the day, this role may be a great fit.
Serve as the primary point of contact for assigned clients and families
Conduct onboarding calls and care assessments to establish initial care plans
Review caregiver visit notes and feedback to identify care issues, gaps, or changes
Coach and support caregivers on client-specific care needs and expectations
Address client concerns and resolve issues related to care quality or consistency
Proactively check in with clients to ensure satisfaction and adjust care as needed
Collaborate closely with Scheduling, Staffing, and Operations teams to ensure care plans are executable
Document all care plan updates, communications, and decisions clearly and consistently
Have worked in a licensed home care agency and understand how care actually plays out in the home
Are comfortable communicating directly with caregivers to provide guidance, feedback, and support
Can handle multiple clients, competing priorities, and frequent context switching
Stay calm when care plans need to change quickly
Are detail-oriented and decisive, not just empathetic
Understand that in home care, things change — and follow-up matters
2+ years experience in a care coordination, client services, or care management role within a licensed home care agency
Direct experience working with caregivers (coaching, problem-solving, or care plan support — not just intake)
Comfortable working remotely in a fast-paced environment
Strong communication skills across phone, email, and internal systems
Bachelor’s degree preferred, but not required.
Competitive pay: $27–$30/hour, based on experience
