The Opportunity
Position Summary:
This Clinical Support Coordinator position is a temporary, grant-funded role in a partnership between Carewest and Health Cities creating a great opportunity for experienced Registered Nurse to apply their skillset in case management and process development.
The role supports the development and implementation of the Right-Housing discharge pathway by providing clinical and operational expertise, assisting with case-finding of ready to discharge residents across Carewest Long Term Care programs, and coordinating site access and workflow reviews.
The role works closely with Health Cities and internal Carewest leaders to support co-design activities, identify risks and improvement opportunities, and ensure compliance with Carewest policies and privacy requirements. It also coordinates the collection and sharing of de-identified data, facilitates staff participation in evaluation activities, and contributes to assessing client needs, discharge barriers, community supports, and the overall viability of scaling the model.
Qualifications & Experience:
- Bachelor’s degree in Nursing.
- Consideration may be given to an equivalent combination of education and experience.
- Active registration with CARNA required.
- 3 to 5 years of relevant clinical experience required.
- 2 to 3 years leadership experience preferred.
- Working in a long-term care environment considered an asset.
