ABOUT THE ROLE
The Director of Care Management provides leadership, supervision, and operational oversight for Wise & Healthy Aging’s Care Management programs serving older adults and their families. The Director ensures high-quality service delivery, supervises care coordinators, supports staff navigating complex client situations, and maintains compliance with funding, documentation, and program requirements.
This role combinesclinical guidance, program management, and team leadership. The Director works closely with staff supporting older adults experiencing complex needs related to health, housing, safety, and family dynamics. The ideal candidate has strong supervisory experience, knowledge of aging services and community resources, the ability to support staff handling emotionally challenging client situations. Clinical licensure such as LCSW, LMFT, LPCC, RN, or LVN, is preferred but not required.

COMPENSATION & BENEFITS
The salary range for the Director of Care Management is $110,000 - $120,000. Wise & Healthy Aging also offers competitive benefits. For staff who regularly work 30 or more hours per week, benefits include up to 80% company-paid medical, 100% company-paid dental, 401(k) plan with employer match, Flexible Spending Account (FSA), optional life and vision insurance, 14 paid holidays plus paid time off (PTO).
