Job Title: Family Medicine Nurse Practitioner

Location: Mesa, AZ

Hours & Schedule: Monday–Friday (full-time outpatient schedule)

Work Environment: Outpatient family medicine clinic serving patients of all ages in a collaborative, team-oriented setting

Salary / Hourly Rate: $110,000 – $130,000 annually (DOE)

Job Summary:

The Family Medicine Nurse Practitioner provides comprehensive primary care services to patients of all ages in an outpatient setting. This role focuses on preventive care, chronic disease management, and acute care while working collaboratively with a multidisciplinary team. The position is ideal for a clinician who values continuity of care, patient education, and meaningful community impact.

Job Duties & Responsibilities:

  • Provide primary care services to pediatric, adult, and geriatric patients
  • Perform physical exams, wellness visits, and chronic disease follow-ups
  • Diagnose and treat acute and chronic medical conditions
  • Order, interpret, and follow up on labs, imaging, and diagnostic testing
  • Provide preventive health counseling and patient education
  • Manage medications and treatment plans
  • Perform minor in-office procedures, including immunizations and wound care
  • Maintain accurate and timely documentation in the EHR
  • Coordinate referrals and care transitions with specialists
  • Participate in team meetings, case reviews, and quality initiatives

Requirements

  • Active, unrestricted Arizona Nurse Practitioner license
  • Master’s degree from an accredited Nurse Practitioner program
  • Current Arizona RN license and national board certification
  • Minimum of two years of outpatient family medicine or primary care experience
  • Experience with EHR systems preferred
