We’re looking for an energetic, highly motivated, results‑driven Registered Nurse (RN) who is passionate about advocating for patients and making a meaningful impact during their most critical moments of care. As a Hospital Clinical Liaison, you will be the essential bridge between acute care and home‑based services—ensuring that every patient experiences a safe, seamless, and empowering transition home.
In this dynamic role, you will build strong, trust‑based relationships with hospital case managers, discharge planners, and social workers to generate referrals for home health, palliative care, hospice, waiver, and private duty services. You’ll meet directly with patients and families, guiding them through care options with clarity, compassion, and confidence.
Your clinical expertise ensures you can accurately review medical records, verify eligibility for services, and help create a smooth, coordinated discharge process that puts the patient’s needs first.

Primary Job Responsibilities:
- Serves as the primary referral contact between assigned hospital partners and KeystoneCare.
- Establishes and fosters professional relationships with case managers, discharge planners, and social workers.
- Performs clinical assessment of potential referrals to determine appropriateness against eligibility criteria.
- Ensures a smooth transition of care from hospital to home by assessing and communicating all patient needs prior to discharge.
- Makes regular, planned visits to assigned facilities and is continually available by phone during business hours.
- Works to build the brands of Home Health, Palliative Care, Hospice, and Private Duty services within the designated market.
- Achieves or exceeds targets for referral volume and growth.
- Demonstrates knowledge of all assigned hospitals and stays abreast of changes and trends in the marketplace.
Requirements
Job Requirements:
- Valid RN license with the state of Pennsylvania
- 3+ years experience as a clinical care coordinator, liaison, or case manager preferred
- Self motivated with excellent time management and communication skills
- Must have clinical knowledge of disease entities common to Home Health and Hospice
- Must have a valid Driver's License and a reliable vehicule
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
