Position Summary for Licensed Vocational Nurse (LVN):

• Position available in San Mateo County.
• Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
• Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager
• Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
• Licensed Vocational Nurse (LVN) positions are also available in San Francisco County.

Minimum Qualifications:

• Current CA Licensed Vocational Nurse (LVN) license
• Current IV  & Blood Withdrawal Certification
• Current CPR card for Healthcare Providers
• 1 year of Licensed Vocational Nurse (LVN) experience
• Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
• Demonstrates excellent verbal, written communication, and organization skills.
