We are recruiting for a number of Healthcare Assistants to join our STARS (Supportive and End of Life Care Service) in Liverpool, North-West.
Hours: Full-time time and Part Time hours available up to 37.5 hours per week (days)
Contract: Permanent
Base: Liverpool Hospice and the community
Salary: £24,465 per annum, in line with the Agenda for Change (AFC) scale & a whole range of Marie Curie benefits
In this role, you will work alongside a multidisciplinary team to provide assessment, care planning, review and evaluation of holistic care management plans individual to each patient.
As this is a community-based role, you will be responsible for planning your own work, responding to changes in patient conditions, prioritising care needs (including hygiene, nutrition and hydration, comfort, monitoring vitals, and more) as detailed in the personalised care plan and ensuring you escalate appropriately to senior staff within the multidisciplinary team in a timely manner.
