Medical Assistant – Senior Care

This in-office role plays an essential part in supporting providers who are out in the field delivering in-home and community-based care. You will help coordinate care, manage follow-ups, and support chronic disease management for older adults.
Working closely with providers who are out in the field, you will help ensure care plans are implemented, referrals and labs are tracked, and patients receive consistent follow-up. This position is ideal for a Medical Assistant with experience in primary care, senior care, or home health who enjoys organization, patient communication, and supporting continuity of care.
Key Responsibilities
Provide administrative and clinical support to field-based physicians and nurse practitioners
Review provider schedules and prepare patient charts prior to visits
Assist with implementation and tracking of care plans for patients with chronic conditions including diabetes, hypertension, COPD, and heart disease
Support patient care coordination by managing follow-ups, referrals, medication updates, and lab tracking through structured communication workflows
Communicate with patients, providers, specialists, pharmacies, home health agencies, and family members to ensure seamless coordination of care
Identify patient concerns, symptom changes, or barriers to care and escalate appropriately
Maintain accurate and timely EMR documentation
Monitor pending labs, referrals, and treatment plans to reduce care gaps and prevent avoidable hospitalizations
Qualifications
Medical Assistant Certificate or Diploma required
Minimum 2 years of experience in primary care, senior care, home health, or care coordination preferred
Experience supporting chronic disease management strongly preferred
Proficient in EMR systems and clinical documentation
Strong organizational and communication skills
Comfortable managing ongoing patient follow-up and coordination tasks
Bilingual English/Spanish preferred
