Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Summary
We are seeking a compassionate and skilled Nurse Practitioner to join our Hospice team on a per diem basis. The Nurse Practitioner (NP) will perform face-to-face visits with hospice patients to determine their ongoing eligibility for hospice care in alignment with CMS regulations. 

Schedule:

Days and hours are flexible

Pay:

$225 per visit

Location:

Marin, Alameda, Contra Costa & San Mateo Counties

By the Bay Health, Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

 

Work Environment
  • Hybrid role with visits in home, facility, and inpatient settings
  • Travel within service region required; valid and reliable transportation needed
Key Responsibilities
  • Conduct hospice face-to-face visits and document eligibility in compliance with CMS regulations
  • Ensure accurate and timely documentation to support quality care and regulatory requirements

 

Education/Experience:

  • Bachelor’s degree and graduate of accredited Nurse Practitioner Program or Master’s in clinical field of nursing or clinical field related to nursing
  • Experience in an acute care, home health, palliative or hospice setting preferred

Certificates, Licenses, Registrations:

  • Advanced practice Registered Nurse with active Nurse Practitioner license
  • Public Health Nursing certification is highly desirable
  • HPNA certification preferred
