Position Details:
- Location: Columbia City, IN
- Schedule: Full-Time, 3rd Shift
- Hours: 10:00 PM – 8:00 AM
- Pay: $15.00 – $18.00 per hour

Responsibilities:
- Assist clients with personal care tasks (bathing, grooming, hygiene)
- Provide companionship and emotional support
- Assist with light housekeeping, meal prep, errands, and mobility
- Perform range-of-motion and mobility exercises per care plan
- Report changes in client condition to the nurse case manager
Requirements:
- Active Indiana HHA license (or CNA with relevant home care experience)
- High school diploma or GED
- 1+ year of experience in home health care
- CPR certification (current)
- Physical exam and negative TB test (within the last 12 months)
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to lift 50 lbs and perform physical duties of the job
- Clear drug test and background screening
- Strong verbal and written communication skills
- HIPAA compliance and commitment to client confidentiality
