Aftercare RN – Join Our Dynamic Healthcare Team

Location: Colombia (Any city) – 100% On-Site Schedule:

• Core Hours: 1 PM – 9 PM EST

• Flexibility: May include weekends within 9 AM – 9 PM EST (subject to operational needs)

About the Role: We are seeking an experienced Aftercare Registered Nurse (RN) to collaborate with physicians, Advanced Practice Providers (APPs), and clinical staff to ensure seamless patient care and compliance. This role involves daily rounds, coordinating referrals, preparing documentation, conducting quality audits, and maintaining CLIA standards.

Key Responsibilities:

• Collaborate with Physicians, APPs, and clinical staff for accurate interpretation and documentation of results.

• Assist with paperwork for Assisted Living Evaluations and other medical documentation.

• Perform chart QA/QI audits as directed.

• Monitor vendor SLA compliance and report discrepancies.

• Participate in daily care coordination rounds and maintain clear communication with all stakeholders.

• Ensure timely follow-up of results with patients, healthcare proxies, and referring parties.

• Support completion of medical paperwork and occasional clinical triage tasks.

Required Skills & Qualifications:

• Active RN license.

• Minimum 1 year of experience in Adult Medicine, Geriatrics, or Adult Urgent Care.

• Strong organizational and communication skills; ability to thrive in a fast-paced environment.

• Knowledge of medical terminology and clinical procedures.

• Proficiency in Microsoft Office Suite and EMR systems (eClinicalWorks preferred).

• Ability to remain calm under pressure and adapt to changing priorities.

• Detail-oriented with a commitment to quality and compliance.
