In this role, you will:
- Complete, coordinate, and transmit MDS assessments in accordance with ICF regulations and required timelines
- Ensure clinical documentation supports resident needs, services provided, and regulatory standards
- Partner with nursing leadership, providers, and interdisciplinary teams to maintain compliance and accuracy
- Track changes in resident status and update assessments as required
- Support person-centered care planning using accurate assessment data
- Prepare for surveys by maintaining organized, compliant records
- Stay current on ICF, state, and federal regulatory requirements

Qualifications
What we need from you:
- Registered Nurse (RN) license in Illinois
- Current MDS 3.0 certification is preferred
- Minimum of 1+ years of MDS experience in a Skilled Nursing Facility
- MOD rotation will be required - approximately 1 weekend every 8 weeks or so

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.

About the Company
- Status: This is a .8 or 32 hour per week position.
- Schedule: 5 days per week, with weekend shifts as needed.
- Salary: based on nursing license, experience and certification/credentials
At our behavioral health center (Frankfort Terrace, an Independent Care Facility or ICF), the MDS Nurse plays a vital role in supporting regulatory compliance while ensuring each resident’s clinical and behavioral health needs are accurately reflected. Your assessments help guide care planning, program services, and reimbursement, all within a recovery-focused, therapeutic environment.
