The Care Coordinator is responsible for providing care coordination services by working collaboratively with a team of individuals involved in the care team as they provide medical, behavioral, psycho/social care, coordinating long-term services and supports, home and community-based services, health and wellness classes, and other related services and supports to Concerto members. Care Coordinators serve as an active advocate for the members’ rights. Plays a key role in the interdisciplinary care team and is a resource to the member, their families, and support systems.
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Travel is required and the percentage of travel is based on patient and enrollment needs.
• The following licenses meet the required qualification for this position and in compliance with MMP Contract:
• Licensed Physician Assistant, Licensed Nurse Practitioner, Licensed Registered Nurse (RN), Licensed Bachelor Social Worker (LBSW), Licensed Master Social Work (LMSW)
• Will consider Limited License Master Social Worker (LLMSW)
• 1-3 years of experience in a healthcare, patient oriented customer service position required
• Experience with the populations similar to the Medicare Enrollee population, including community-based and facility-based people with medical, LTSS and/or behavioral needs
• Case management experience preferred
• Disease management experience preferred
