Schedule: Full-Time, On-Call schedule is from Monday-Thursday 5p-8a.

Responsibilities:
- Respond promptly to on-call requests, triaging patient needs via phone and in-person visits.
- Assess patient condition and provide skilled nursing interventions for pain and symptom management.
- Educate patients and families on hospice care, medication administration, and comfort measures.
- Document all patient interactions accurately in the electronic medical record (EMR) per regulatory and agency standards.
- Communicate with interdisciplinary team members regarding changes in patient status.
- Coordinate with pharmacies, DME providers, and other resources as needed.
Requirements:
- Active RN license in the State of Texas
- Hospice 1-2yrs experience preferred
- Strong organizational, assessment, and time-management skills
- Excellent communication and customer service skills
- Valid driver’s license and reliable transportation
