Licensed Practical Nurse (LPN) - Weekend Day Shift

What you will do in this role: 

As a Licensed Practical Nurse (LPN) you will be joining a team of nursing professionals dedicated to providing excellent clinical care.  Some have been with us for a long time and can mentor others.  You will help patients reach their clinical goals and work side by side with a highly skilled multidisciplinary team. 

• Our DON is hands-on, eager to help out where needed and willing to train.

Qualifications

What we need from you:

• Licensed Practical Nurse (LPN) licensed in IL
• Prior experience in a SNF or ICF preferred 
• Prior experience with the long term memory care patient
• Electronic medical records experience is helpful (Point Click Care or PCC is ideal)

This center is managed by TRANSITIONAL CARE MANAGEMENT.  We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.

About the Company

• Salary:  LPN $36/hr.
• Schedule:  Every Saturday and Sunday, 8 hour shifts

The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit.  This close-knit team is welcoming and always willing to jump in to help out when you need it.  What makes us unique:
