- Provide safe therapeutic care in holistic and systemic way to include the assessment and overall management of care for oncology and palliative care patients both in a clinic and home-based setting.
- Follows standards of care as outlined by Vail Health and Colorado Nurse Practice Act.
- Exercises independent judgement in the assessment, diagnosis and initiation of medical processes and procedures.
- Communicates effectively and works collaboratively with others.
- Demonstrates respect for and understanding of other clinical disciplines and uses an integrated approach to achieve desired patient outcomes.
- Utilizes evidenced-based standards, guidelines, and protocols for care delivery.

- Patient care duties may include but is not limited to comprehensive physical assessment, clinical recommendations for symptom management, ordering and interpretation of laboratory results, goals of care discussions, advance care planning, education and research.
- Prescribes medications in accordance with state and federal guidelines, including controlled substances.
- Participates in Quality/Best Practice initiatives by utilizing standards and guidelines for cost-effective care delivery in order to continuously improve patient care and enhance outcomes.
- As an integral member of the Shaw Cancer Center team, collaborates routinely with both medical oncology and radiation oncology practices.
- Attends weekly chart rounds, routine staff meetings, Commission on Cancer (CoC) and other accreditation committees.
- Contributes to the ongoing development of the Shaw at Home program through the creation of policies, protocols, and guidelines.
- Contributes to data collection, entry, assessment, and reporting as needed.
- Participates in routine palliative care Interdisciplinary Team (IDT) meetings for debriefing, clinical case review, and ongoing education.
- In conjunction with scheduler, maintains and manages patient scheduling, organization of daily schedule, to include prompt response to patient outreach by phone/email.
- Does so by maintaining responsible oversight of the balance of in-home visits, clinic visits, and administrative time.
- Completes accurate and timely documentation within Electronic Health Record (EHR).
- Available for after-hour emergencies .
- Role models the principles of a Just Culture and Organizational Values.
- Must be HIPAA compliant
- Two years' experience as a nurse practitioner in a related area preferred.
- Two years of oncology experience preferred.
- Palliative and/or Hospice experience preferred
- Required graduation from an accredited nursing master’s program required.
- Required ability to maintain a State of Colorado Nurse Practitioner license with prescriptive authority
- Drug Enforcement Administration (DEA) license required
- Valid Driver's license and ability to pass Vail Health's Department of Motor Vehicle Report criteria required
- Certification by the American Nurses Credentialing Center (ANCC) or National Certification Corporation (NCC)
- Advanced Oncology Certified Nurse Practitioner (AOCNP) preferred
- Basic Life Support (BLS) required
