Health Link Home Health & Hospice is a family-owned agency that has been serving the Bay Area for nearly 20 years, and we’re looking for a compassionate Nurse Practitioner to join our team.
In this role, you’ll provide palliative and supportive care to patients in their homes, helping manage symptoms, improve comfort, and support patients and families through serious illness.
This is a great opportunity for a Nurse Practitioner who enjoys clinical autonomy, meaningful patient relationships, and strong team support. Our experienced San Francisco office team handles scheduling and coordination, allowing you to focus on providing thoughtful, patient-centered care.
Visit patients in their homes throughout the San Francisco Bay Area
Perform health assessments and physical exams
Develop care plans focused on comfort, symptom management, and quality of life
Prescribe and adjust medications within your scope of practice
Support patients and families through serious illness and advanced care planning
Work closely with our interdisciplinary care team including nurses, physicians, and social workers
Document visits and care plans in the electronic health record
Required
Active California RN License
Certified Nurse Practitioner (NP)
Strong clinical assessment and decision-making skills
Compassionate communication with patients and families
Ability to work independently in a home care setting
Willingness to travel to patient homes throughout the Bay Area
Preferred
Experience in palliative care, oncology, geriatrics, or complex chronic conditions
Experience collaborating with interdisciplinary care teams
