Full-Time, Part-Time, or Per Diem

Health Link Home Health & Hospice is seeking an Occupational Therapist (OT) to support our growing South Bay team.

We provide the structure and operational support therapists need to focus on patient care — organized scheduling, strong clinical coordination, and modern documentation tools.

Your Role

• Perform occupational therapy evaluations and reassessments
• Develop and manage individualized plans of care
• Deliver skilled therapy focused on ADLs, safety, fall prevention, and independence
• Address home safety and adaptive equipment needs
• Educate patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Complete documentation in Kinnser

Requirements

• Active California Occupational Therapist license
• Valid California driver’s license
• Strong clinical judgment and time-management skills
• Home health experience preferred but not required
