As our organization has grown, so has the need for a dedicated Patient Liaison with strong clinical judgment, exceptional organizational skills, and a deep understanding of family-centred care. We are seeking a registered Social Worker who thrives at the intersection of patient support, clinical coordination, and operational excellence.

This is an office-based role, Monday to Friday. This role is ideal for a Social Worker who enjoys working at the systems and coordination level—supporting families through complex transitions, anticipating risk, and ensuring care teams are thoughtfully matched, prepared, and supported. You will have a meaningful influence on patient experience, staff readiness, and care quality without carrying a frontline caseload. 

About the Role

The Patient Liaison plays a critical role in ensuring that patients with complex needs and their families are matched with well-prepared, well-supported nursing teams. You will work closely with Ontario Health atHome (OH@H), internal clinical leaders, and families to coordinate care, prepare staff, and support quality and risk management processes.

This role also supports workforce readiness and quality assurance initiatives, helping ensure that nurses are trained, confident, and aligned with each family’s unique needs.

Key Responsibilities

Patient Services & Program Coordination

  • Act as a consistent point of contact for families navigating complex care needs, transitions, and service changes.
  • Support family-centered planning, communication, and escalation for high-risk or high-complexity cases
  • Manage complex care nursing programs, including paediatric and adult clients, in partnership with Ontario Health atHome (OH@H).
  • Build and maintain strong relationships with Care Coordinators and external stakeholders
  • Source, match, and prepare nurses for existing and prospective clients
  • Coordinate client-specific training, including SickKids modules, skills labs, and shadow shifts
  • Attend OH@H meetings and bi-weekly huddles
  • Assist with incident reviews, investigations, and formal reporting
  • Coordinate supply ordering and inventory related to complex care programs

Staff Preparation, Quality & Compliance

  • Support onboarding and orientation of new nursing staff
  • Ensure compliance with certifications, annual training, and documentation requirements
  • Coordinate ongoing education (e.g., GPA, skills refreshers, specialized training)
  • Review and action operational reports (progress notes, documentation quality, attendance)
  • Support performance management processes in collaboration with leadership
  • Participate in Quality Assurance, Risk Management, and Health & Safety initiatives

Administrative & Team Support

  • Support scheduling during peak periods or coverage gaps
  • Respond to intake and service inquiry calls as needed
  • Contribute to the continuous improvement of workflows and patient experience

Requirements

  • Bachelor of Social Work (BSW) or higher (MSW preferred)
  • Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • 3–5 years of experience in healthcare, hospital, home care, or community-based settings
  • Strong understanding of complex family systems and high-acuity care environments, including pediatrics and/or adult complex care
  • Exceptional organizational and time-management skills
  • Excellent interpersonal communication and professional judgment
  • Comfort working in-person as part of a collaborative office-based team
