Impact Life is a recovery-oriented nonprofit organization providing whole-person, trauma-informed care across Delaware. Through our Social Prescribing Program, we connect individuals to community-based resources that address social determinants of health, including housing, employment, food access, education, and recovery supports.
We are seeking a Peer Link Worker to support participants in identifying goals, navigating community resources, and building pathways toward stability and wellness. This role combines social prescribing principles with peer recovery support and is ideal for someone who is passionate about advocacy, community connection, and person-centered care.
What You’ll Do
As a Peer Link Worker, you will:
- Build trusting relationships using a trauma-informed, strengths-based approach
- Conduct needs and strengths assessments with participants
- Co-develop individualized goal plans focused on health, stability, and wellbeing
- Connect participants to community resources such as housing, benefits, employment, recovery supports, and wellness activities
- Support warm handoffs and coordinated referrals
- Advocate for participants while promoting autonomy and self-advocacy
- Maintain timely documentation and track engagement and outcomes
- Collaborate with clinical teams and community partners
- Participate in supervision, case reviews, and team meetings
Schedule
Full-time, on-site position based in Newark, DE.
- Monday through Friday, 8:00 AM – 4:00 PM
- Occasional evenings and weekends required based on participant and program needs
- Local travel within New Castle County required
Requirements
Who You Are
Required
- Eligible to become a Certified Peer Support Specialist in Delaware
- Demonstrated lived experience in recovery and ability to share appropriately
- Strong communication and relationship-building skills
- Ability to work independently and as part of a multidisciplinary team
- Basic documentation and technology skills
Preferred
- Current Peer Specialist certification
- Experience in behavioral health, outreach, case management, or community services
- Knowledge of local community resources
- Experience working with underserved or justice-involved populations
- Bilingual skills
