Your Role

• Independently perform physical therapy evaluations and reassessments.

• Develop and manage individualized plans of care.

• Deliver skilled therapy focused on mobility, function, fall prevention, and independence.

• Supervise and collaborate with PTAs as applicable.

• Communicate directly with physicians and the interdisciplinary team.

• Educate patients and caregivers.

• Complete documentation in Kinnser supported by AI tools.

• Serve as the primary PT voice in patient care planning.

Requirements

• Current California Physical Therapist license.

• Valid California driver’s license.

• Strong clinical judgment, organization, and time-management skills.

• Home health experience is preferred, but strong outpatient, inpatient, or rehab PTs are encouraged to apply.
