We are strengthening our East Bay team and are seeking a Physical Therapist who wants flexibility, independence, modern tools, and an organized home health structure behind them.

Flexible Scheduling
Independently perform physical therapy evaluations and reassessments
Develop and manage individualized plans of care
Deliver skilled therapy focused on mobility, function, fall prevention, and independence
Supervise and collaborate with PTAs as appropriate
Communicate directly with physicians and the interdisciplinary team
Educate patients and caregivers
Complete documentation in Kinnser, supported by AI tools
Serve as the primary PT voice in patient care planning
Current California Physical Therapist license
Valid California driver’s license
Strong clinical judgment, organization, and time-management skills
Home health experience preferred
Strong outpatient, inpatient, or rehab PTs are encouraged to apply
