Registered Nurse - Home Health Experience

Cheer Home Care is looking for a Registered Nurse who loves spending time with older adults and provide high quality care in home. The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. .

This position responsibilities will include:

Complete assessment of client and needs

Education to client and families

Chronic Care Management

Develop care plan and establish goals

Post-surgical and wound care as needed

Wellness support

Requirements

• Upbeat Personality
• Active California RN License
• Industry Experience, required
• Strong Computer Skills
• Ability to multitask, drive, take action
• Pass Live Scan Fingerprint Clearance
• Valid driver’s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results

$55-$70 an hour based on experience. Potential for team bonus.
