Brellium's mission is a big one – to improve the standard of care across the US healthcare system. We’ve built AI-powered technology that helps healthcare providers deliver safer, higher-quality care - starting with the first real-time medical review platform built to fix clinical and compliance risks before they impact patients.
Each year, 1 in 20 people in the U.S. experiences a medical diagnostic or compliance-related mistake. Most providers lack the time, staffing, and tools to mitigate these issues - so they go unnoticed, impacting care quality and increasing clinical and financial risk.
Brellium is building the AI-powered platform that helps providers deliver safer, more consistent care by mitigating risk early and aligning patient visits with clinical best practices. Our goal is to give every provider in the U.S. the tools to deliver clinically excellent, data-driven care - at scale.
Brellium was founded in 2021. Since then, we’ve grown to serve over 250,000 providers across all 50 states who use Brellium to take better care of their patients and ensure data-driven, compliant care. We’re a Series A company with over $30MM in funding from First Round Capital, Left Lane Capital, and Menlo Ventures.
We’re hiring Senior Software Engineers across three distinct archetypes. All three share the same bar:
You build high-quality systems with strong ownership.
You operate with autonomy.
You care about craft and outcomes.
You want to build foundational infrastructure in a high-growth environment.
Proficiency with modern backend development (Python/TypeScript) and cloud infrastructure (AWS). We value strong engineering fundamentals and are open to candidates from other language ecosystems.
Depending on your strengths, you’ll focus on one of the following areas:
Archetype 1: AI Native Engineer
Own and evolve the intelligence engine powering Brellium.
You’ve built with LLMs beyond toy demos. You understand prompt design, evaluation frameworks, agentic systems, structured extraction, tool use, retrieval pipelines, and performance tradeoffs. You’ll work directly on the core engine that transforms raw session data into clinically structured outputs — improving accuracy, latency, reliability, and cost efficiency at scale. What You’ll Do
Design and improve LLM-powered pipelines and agent architectures
Build robust evaluation and benchmarking frameworks
Optimize for latency, cost, determinism, and output quality
Implement retrieval and tool-use strategies where appropriate
Ship production-grade AI systems — not experiments
You Might Be a Fit If You:
Have built and deployed LLM systems in production
Understand modern agent patterns (tool calling, RAG, memory, orchestration)
Think deeply about evaluation, hallucination mitigation, and reliability
Care about performance and operational excellence, not just capability
Are excited to build a core engine that operates at massive daily volume
Archetype 2: Backend / Infrastructure Native Engineer
Design the systems that let Brellium scale 10x.
We currently process ~100k sessions per day. We’re building toward 1 million+. That requires robust distributed systems, resilient pipelines, and infrastructure that doesn’t break under load. You'll architect and build the backend systems that ensure reliability, observability, and performance as volume increases by an order of magnitude. What You’ll Do
Design and evolve distributed systems to support 10x growth
Improve throughput, fault tolerance, and system resilience
Build scalable data pipelines and processing infrastructure
Strengthen observability, monitoring, and incident response
Reduce operational bottlenecks and technical debt
You Might Be a Fit If You:
Have scaled systems handling high throughput or real-time processing
Are comfortable with distributed architectures and failure modes
Think in terms of system design, not just features
Have strong backend fundamentals (databases, queues, concurrency, APIs)
Care deeply about reliability and performance under load
Archetype 3: Product Native Engineer
Ship meaningful products end-to-end.
You’re comfortable across the stack and thrive at the intersection of engineering, product, and users. You can take a feature from ideation through architecture, implementation, polish, and launch. You’ll work closely with leadership and customers to turn ambiguous problems into shipped product.
What You’ll Do:
Translate product ideas into technical designs
Build features across frontend, backend, and data layers
Collaborate directly with design and leadership
Own projects from concept to production
Make pragmatic tradeoffs between speed and craft
You Might Be a Fit If You:
Have shipped full-stack features independently
Are product-minded and user-obsessed
Operate well in ambiguity
Care about velocity and quality
Enjoy wearing multiple hats
What All Three Roles Share Regardless of Archetype, you:
Have 5+ years of professional software engineering experience
Write clean, maintainable code
Think in systems, not just tasks
Operate with ownership and accountability
Want to build something foundational in healthcare
Compensation: $190,000-$260,000

Bias to Action: Brellium teammates do not wait to make reversible decisions or seek unnecessary approval. We quickly decide and move forward. If the decision was incorrect, we quickly reverse it and move forward.
Thinks for themselves: Brellium teammates do not take things at face value. We ask "why" until base truth is reached. If a better solution is present, Brellium teammates use it, regardless of status quo.
Negative Maintenance: The opposite of high maintenance isn’t low maintenance - it’s negative maintenance. Brellium teammates are poised under pressure, self-motivated, self-improving, self-disciplined, self-aware, and non-defensive.
Expect Excellence: We hold ourselves to exceptionally high and continuously rising standards. We strive for thoughtfulness in our decision making, and for speed and quality in our execution. We acknowledge trade-offs and communicate proactively.
Communicate with Clarity: Brellium teammates communicate concisely, directly, and purposefully. We optimize for ensuring our points are easily understood the first time.
