Today, patients and clinicians feel the time, energy, and stress wasted on the administrative burden of the U.S. healthcare system. At R1, we are building an AI-powered revenue operating system to hide that complexity—helping hospitals get paid accurately and faster so clinicians can focus on care instead of paperwork.
You’ll join R37, R1’s AI lab, where engineers and scientists build production AI and agentic systems that automate medical coding, billing, and denial management. This is not academic research or disconnected experimentation—your work directly powers systems used by 95 of the top 100 hospital systems in the U.S.
R37 combines the pace and ambition of a startup with real revenue, clear business value, and access to one of the richest healthcare datasets in the world.
We work with massive, real datasets:
180M+ claims
550M patient encounters
1.2B workflow actions, and their ultimate outcomes
Your work will directly impact how quickly hospitals get paid—and how effectively AI can reason over complex, unstructured healthcare data.
What you'll do
You’ll lead the development of early-phase, high-impact AI systems and help bring scientific rigor into production environments:
Design and develop AI systems for retrieval, ranking, categorization, and generative AI over large-scale unstructured healthcare data
Own AI projects end-to-end: from problem framing and model selection to production deployment and ongoing improvement
Work backwards from complex business problems to define AI abstractions and system architectures that are scalable, explainable, and maintainable
Bring rigor to scientific decisions by defining the right evaluation datasets, metrics, and validation strategies tied to real outcomes
Build feedback loops and data flywheels that continuously improve model performance in production
Partner closely with software engineers, product managers, clinicians, and operators to ensure AI systems deliver measurable business value
Mentor other AI engineers and promote best practices across modeling, evaluation, and deployment
Contribute to hiring and interview loops to help grow a world-class applied AI team
Examples of problems you may work on
Developing retrieval-augmented and generative AI systems that reason over complex clinical and payer documentation
Designing ranking and triage models that determine how work is routed between AI agents and human operators
Building evaluation frameworks that connect model outputs to downstream financial and operational outcomes
Prototyping and scaling new AI capabilities from 0 → 1, then hardening them for real-world production use
Who you are
AI Engineer with a PhD and 6+ years of post-PhD experience, or equivalent industry experience at scale
Proven track record building and operating production AI systems (e.g., search, ranking, recommendation, or generative AI)
Comfortable moving between research, prototyping, and production engineering
Strong ability to translate ambiguous business problems into concrete modeling and system decisions
Experienced across the full AI lifecycle: data exploration, model development, evaluation, deployment, and iteration
Hands-on with modern ML tooling (e.g., PyTorch, TensorFlow, Hugging Face) and production platforms (e.g., Databricks, AWS Bedrock, Azure ML)
Thoughtful mentor who raises the scientific and technical bar of the team
Motivated by real-world impact, not just offline metrics
Healthcare experience is not required.
Why AI Engineers choose R37
Applied AI at uncommon scale with real operational and financial impact
Direct ownership of AI systems that run in production—not just experiments
Rich, high-signal datasets rarely available outside healthcare
Close collaboration with strong software engineers, clinicians, and operators
A fast-growing organization with a path to expand responsibility and impact
Serious, high-impact work—with a team that values rigor, curiosity, and collaboration
Interview Process
Remote interviews focused on applied AI problem solving, system design, and scientific decision-making, followed by a final in-person interview at one of our hubs (SF, NYC, Austin, or Chicago; travel arranged).
Interview Logistics Notice
As part of our hiring process, selected candidates will participate in an in-person interview. Candidates located near one of our talent hubs—San Francisco, New York, Austin, or Chicago—will be scheduled to meet with team members in those locations. For candidates residing outside these areas, we will arrange travel to a hub for the interview. Travel accommodations will be provided as needed. We are committed to providing equal employment opportunities and ensuring a fair and inclusive experience for all applicants.
For this US-based position, the base pay range is $120,000 to $300,000 per year. Individual pay is determined by role, level, location, job-related skills, experience, and relevant education or training.
This job is eligible to participate in our annual bonus plan.
The healthcare system is always evolving — and it’s up to us to use our shared expertise to find new solutions that can keep up. On our growing team you’ll find the opportunity to constantly learn, collaborate across groups and explore new paths for your career.
Our associates are given the chance to contribute, think boldly and create meaningful work that makes a difference in the communities we serve around the world. We go beyond expectations in everything we do. Not only does that drive customer success and improve patient care, but that same enthusiasm is applied to giving back to the community and taking care of our team — including offering a competitive benefits package.
