Healthcare Practice Transformation Sr. Manager

What We Need:

 The Oregon Primary Care Association (OPCA) seeks a Practice Transformation Senior Manager to coordinate and handle day-to-day activities related to process improvement at community health centers in Oregon and SW Washington alignment with the Mountain West AIDS Education and Training Center (AETC) & Oregon’s End HIV initiative.

The Practice Transformation Senior Manager works directly with Federally Qualified Health Centers (FQHCs) across the state to improve health outcomes along the HIV care continuum and coaches for practice change, with an emphasis on quality improvement initiatives that improve access for people experiencing systemic barriers to care. Using an implementation science framework, this position bridges the gap between best and promising advances in clinical practice, social determinants of health, and systems level interventions needed to support positive health outcomes in Oregon.

Who You Are:

A highly skilled facilitator with strong project management skills to manage work plan details for HIV/STI Program capacity, building assistance activities at FQHCs and coordinating healthcare provider trainings, conferences & workshops to support practice change efforts within FQHCs that support positive health outcomes for patients.

Your ability to think strategically and deliver technically allows you to combine an understanding of high level organizational and team goals with an impeccable attention to detail in your daily work. As a collaborative relationship builder, you work closely with your team and other thought partners.

What You’ll Bring:

• Knowledge/understanding of practice transformation, implementation science, and the building blocks of high performing primary care.
• Knowledge/understanding of HIV/AIDS educational program and priorities, including early intervention activities and disease investigation.
• Knowledge of Federally Qualified Health Center metrics, reporting and day-to-day clinic operations/workflows.
• Proficiency withprogram management tools (e.g., Monday, Asana) to coordinate work and communicate effectively across teams.
• Experience working with webinar and teleconference software like Microsoft Teams, Zoom.
• Familiarity with assessment tools such as flow charts, swim lanes, gantt charts, etc.
• Solid communication and interpersonal skills to communicate effectively with all levels of staff, both verbally and in writing.
• Solid ability to multi-task with demanding timeframes.
• Superior ability to prioritize assignments, manage several concurrent projects effectively with frequent interruptions under deadlines.
• Ability to use sound judgment in responding to issues and concerns both internally and with external community partners.
• Ability to travel to support training and capacity building assistance in alignment with program deliverables.

What You’ll Do At OPCA:

Technical Assistance and Training Program Management

• Explore and evaluate opportunities for program growth to address prevention screening, linkage to care and treatment, and viral suppression of HIV in the context of workforce and system development along the HIV Care Continuum.
• Apply knowledge and technical expertise to assist FQHCs in developing and implementing problem-solving approaches to the complex challenges experienced by people with HIV and those at increased risk of acquisition.
• Market and facilitate learning opportunities (training events, communities of practice, webinars, conferences) for health systems in Oregon that support practice change.
• Coordinate onsite training activities related to program deliverables.
• Implement quality improvement (QI) projects to improve sexual health history taking; HIV and sexually transmitted infection (STI) screening and treatment as well as linkage to care and viral suppression.

Member and Partner Engagement

• Coordinate with community partners, FQHCs clinic administration and public health to design and present responsive curricula to address gaps in services and improve access to quality healthcare services.
• Build relationships with FQHCs and work with clinics to identify key practice improvements along the HIV care continuum that align with clinic goals.
• Provide capacity-building assistance to community partners around access and linkage to care along the HIV Care Continuum.
• Build and maintain relationships with FQHCs and collaborating partners to identify training opportunities in alignment with Oregon public health goals

Program and Resource Tracking, Evaluation and Reporting

• Ensure appropriate documentation and reporting of training outcomes in compliance with funder requirements.
• Assist with program evaluation efforts and quality improvement practices, including data entry and reporting on program deliverables in alignment with existing contracts.
• Apply a critical lens to both internal and external policies, practices, and procedures to support continuous quality improvement along the HIV Care Continuum.

Other Duties As Assigned

Minimal Qualifications:

• Professional with advanced level of proficiency. Generally requires a Bachelor’s degree
• and/or 5+ years’ experience or equivalent education. Broad knowledge of the field with proven leadership skills.
• Fluency in written and spoken English required.

Starting Range:

$75,000 - $82,000

OPCA works a hybrid schedule of two days in our downtown office and three days remote.
