Registered Nurse - Home Health
Position requirements
- Performs assessments and develops individualized patient care plans and manages multi-disciplinary care as applicable.
- Provides skilled nursing care in a patient’s home as ordered by the attending physician in conjunction with the plan of care.
- Supervises LPN’s and Home Health Aide’s in a patient’s home.
- Complies with agency policies and procedures.
- Promotes and maintains an agency environment that is in compliance with federal, state, and local regulations.
- Submits clear and appropriate documentation in a timely manner and documents patient education.
- Communicates effectively to physicians, supervisors, and coworkers in a timely manner.
- Participates in case conferences to promote coordination of care.
- Promotes continuity of care with appropriate admissions, transfers and discharges.
- The RN will attend in-service training and attend mandatory agency meetings.
- Reports to appointments as scheduled.
- Maintains a professional appearance
- Perform all aspects of patient care.
Qualifications
- Registered Nurse with current license in the state of Illinois, required.
- Registered Nurse with current license in the state of Wisconsin, preferred.
- One year of experience as an RN in an acute care setting, preferably two years of experience.
- Must have at least one year of Home Health Experience.
