Position Title: Family Planning Coordinator

Position summary:
Responsible for providing case management, family planning, and health education to patients.

Responsibilities:
- Conduct obstetrical, nutrition, health education and psychosocial assessments, and
develop an Individualized Care Plan (ICP) with perinatal patients following
Comprehensive Perinatal Services Program (CPSP) protocols.
- Manage perinatal patients, closely monitoring attendance at appointments, status of
referrals, identifying and addressing barriers to care, and providing support and resources
as needed.
- Act as a liaison between the clinical team and other community agencies to establish an
integrated and coordinated program.
- Maintains a referral network through local partnerships or community agencies in order
to enhance the services provided based on patients’ needs.
- Assists clients with access to Medi-Cal by providing Presumptive Eligibility for Pregnant
Women (PE4PW) per Medi-Cal guidelines.
- Coordinate the transfer of records to/from the hospital for continuity of care.
- Coordinates and assists perinatal patient in scheduling newborns first visit.
- Coordinate appointments, test/procedure scheduling, insurance eligibility and authorizations,
responding to patient calls, appointment reminder, scheduling, and maintaining information as
it relates for the surgery coordination between assigned OBGYN providers and the hospital.
- Participates in trainings provided by the CPSP program to further education and keep
current with industry changes, and clinical requirements.
- Assists perinatal patients to transition to family planning services or to CMOAR primary
care provider and/or other services as needed.
- Provide family planning counseling to all patients requesting services based on sound
knowledge and understanding. This includes basic anatomy and physiology, sexual
development, human reproduction, value of fertility regulation, pregnancy testing, safe
sex practices, HIV/AIDs risk assessment and prevention/risk reduction, routine
reproductive health and health promotion/disease prevention, including: screening recommendations, nutrition, exercise, smoking cessation, alcohol and drug abuse,
domestic violence and sexual abuse, and referral to appropriate additional services.
- Counsel family planning patients with sensitivity to their need and desire for information,
with attention to cultural norms and needs, in a non-judgmental and respectful manner at
all times.
- Assist family planning clients to explore feelings and concerns about sexuality, lifestyle
options and choices. Assist clients to make informed decisions about these issues by
providing oral and written information, and help clarifying values and priorities.
- Conduct in reach/outreach presentations regarding CMOAR’s prenatal program including
CPSP and Family Planning services.
- Translate as needed during outreach, education sessions, and presentations.
- Prepare and disseminate family planning educational and informational materials.
- Maintain current knowledge base on family planning, pregnancy, STI/HIV related health
issues.
- Attend/help organize community workshops, trainings and health/resource fairs at the
clinic.
- Assist clients with practical arrangements such as: transportation, and interpretation
needs.
- Call patients to reschedule appointments and follow up on missed appointments
- Ensure timeliness of services rendered to patients and look after their comfort while on
premises.
- Work evenings and weekends when necessary.
- Other duties as assigned within the job classification.
Qualifications/Requirements
- Excellent verbal and written communication skills; bilingual English/ Spanish
- High school diploma, some college preferred
- Typing a minimum 35 WPM and proficient computer skills; including but not limited to
Microsoft products and use of outlook
- Ability to handle multiple tasks and work in a busy environment within the job
classification
- Basic understanding of government programs related to services provided
- Able to work and communicate effectively with people of diverse culture, education and
economic backgrounds
- Family Planning Health Worker (FPHW) and Comprehensive Perinatal Health Worker
(CPHW) certificates and maintain certification.
- Customer service skills: communication, empathy, patience, and technical knowledge
- Work in team-oriented environment, and work well under deadlines.
- Ability to handle multiple tasks and work in a busy environment
- CPR Certification
- Ability to work at multiple clinic sites.
