The Supportive Living Program is a PRP which services individuals in Anne Arundel County experiencing serious behavioral health disorders. We provide symptom management and case management. This is tailored to meet individual's needs, as well as goal work such as physical health, budgeting, daily living skills, social skills, some vocational assistance, and coping strategies.

Persons Served receive at least 6 visits and or services per month from their assigned rehabilitation counselor or through Day Program groups.
Job Description
This position requires direct care work with individuals diagnosed with mental illness – rehab staff will hold a caseload of 15 people for whom staff would assist with case management and goal work that is individualized to that person’s needs 6 times per month (i.e. physical health, symptom management, independent living skills, etc.).
Education
Required High School Diploma or GED, Preferred Associate's or Bachelor's Degree in a related field such as psychology, social work, counseling, or human services. Experience with clinical documentation, including progress notes, service plans, and discharge summaries, is often required. Computer literacy: Proficiency in using a computer for documentation, email, and electronic health records (EHR) systems.Travel: Ability to travel frequently within the service area to meet clients in their homes and communities.
Organizational Skills: Strong ability to manage a caseload, prioritize tasks, and maintain organized client files.
Crisis Management: Basic skills in de-escalation techniques and knowledge of procedures for handling behavioral or psychiatric crises.
Interpersonal & Communication Skills: Exceptional ability to establish rapport and maintain therapeutic relationships with diverse populations. Excellent verbal and written communication skills.
